Credentialing Process

We appreciate the interest of each physician, professional, and vendor who applies for HSCSN/NET provider status. Those interested may request an application packet from Provider Operations by calling 202-467-2732.

HSCSN/NET's credentialing staff will thoroughly review each application packet for completeness and will notify applicants if additional information is needed. If the application is not complete within 90 days of notification, the file and application will be deactivated. If your file and application are deactivated, further consideration for HSCSN/NET provider status will require re-application.
