Health Services for Children with Special Needs, Inc. (HSCSN) provides innovative care to the pediatric Supplemental Security Income (SSI) and SSI-eligible populations of Washington, DC through a management network that coordinates health, social, and education services for our members.
Each member has a care manager responsible for coordinating access to primary and specialty care, developing an individualized care plan, and educating the family on how to best prepare for care transitions. Our approach begins with a thorough screening to determine health care needs. A clinical assessment is performed and an environmental scan is conducted to check for things such as lead in the home. Plus, an assessment is made of the social aspects of a member’s environment that may affect health. This in-depth analysis helps us to personalize each individual’s care management plan.
