Care Coordination
HSCSN/NET provides comprehensive care coordination for every member. Care managers, Member Services representatives, and Community Outreach staff work closely to make certain that all members receive health care and other related assistance. Care coordination begins immediately after DC Medicaid approves an enrollee for membership. It continues until the member is no longer in the program.
The CARE MANAGER is the member's partner in coordinating all needed health care services. Every member has a care manager who is assigned according to the member's needs. Care managers have education and experience specifically tailored to helping children and teens with special needs. They provide expert coordination assistance. The assigned care manager gets to know the member's health needs and goals and works closely with the member's Primary Care Physician (PCP) and specialty providers to ensure all health needs are met. Care managers also work with other public agencies that are involved with the care of members, such as DC Public Schools.
Anytime a member's care manager is unavailable, MEMBER SERVICES (202-467-2737) is there to help 24 hours a day, 7 days a week, every day of the year. Member Services can arrange for transportation for member health care visits, answer questions about HSCSN/NET, and find an interpreter for callers who do not speak English. Member Services also maintains the TTY for Members with hearing impairments.
